Sogenannte Stroke Units, bei denen in spezialisierten Kliniken eine individuelle und konsequente Behandlung durch intensive Überwachung und kurze Behandlungswege gewährleistet wird, verringern das Risiko von schweren Folgeschäden durch Schlaganfälle. Die zuletzt in der Fachzeitschrift Neurology veröffentlichte Studie macht deutlich, dass das Netz von Stroke-Unit Einheiten vergrößert werden muss und jedes Krankenhaus dies als Standard einführen sollte.
Bei einem Verdacht auf Schlaganfall müssen auf schnellem Weg eine intensive Überwachung, die nötigen Untersuchungen in Zusammenarbeit mit Ärzten aus verschiedenen Fachrichtungen und eine umfassende Diagnostik in engmaschigem Ablauf erfolgen. In der Fachzeitschrift Neurology veröffentlichten Studie, die unter der Federführung der Neurologischen Universitätsklinik Heidelberg durchgeführt wurde, machen die Autoren klar, dass die gezielte Behandlung durch eine Stroke Unit Todesfälle und schwere Behinderungen abwenden kann. In der Studie der baden-würtembergischen AG Schlaganfall, der Stelle Qualitätssicherung im Krankenhaus (GeQiK) haben die Unikliniken Mannheim, Heidelberg und Freiburg mitgewirkt. Zwar sind es immerhin 17 Prozent der Patienten in Baden-Württemberg die rechtzeitig handeln, das heißt innerhalb von 4,5 Stunden nach Beginn der Symptome in der Klinik eintreffen, jedoch werden sie ohne Stroke Unit behandelt.
Außerdem sind es mit 60 % der Schlaganfall-Patienten immer noch sehr viele, die für eine wirksame Behandlung verspätet eintreffen und eine Thrombolyse nicht mehr effektiv umgesetzt werden kann. In den Stroke Units werden optimale Überwachung, rechtzeitige Behandlung durch die kurzen Wege zwischen den Fachärzten und schnelle Indikationen und Entscheidungen gewährleistet. Krankenhäuser mit einer fehlenden Stroke Unit setzen die Lysetherapie seltener ein als spezialisierte Kliniken. Individuelle Maßnahmen bleiben aus. Das hat schwerwiegende Folgen, denn die Autoren betonen, dass die fachgerechte Lysetherapien die durch die Stroke Units ermöglicht werden, hunderte Patienten pro Jahr vor Folgeschäden bewahren würde. Daher erhofft sich Studienleiter und Leiter der Heidelberger Stroke Unit Professor Dr. Peter Ringleb, dass Rettungsdienste in solchen Fällen konsequent Krankenhäuser mit Stroke Unit ansteuern. Ausgewertet wurden unter der Leitung von Professor Dr. Ringleb und Dr. Christoph Gumbinger von der Neurologischen Uniklinik Heidelberg, Daten die Hinweise über stationäre Behandlungen von Schlaganfallpatienten in Baden-Württemberg von 2008-2012 geben. Demnach waren es 40 Prozent der Patienten die innerhalb von 4,5 Stunden nach Eintreten der Symptome für eine Lysetherapie in der Klinik eintrafen. Bei 44 Prozent dieser Patienten wurde umgehend eine Thrombolyse eingeleitet. „Das klinische Ergebnis dieser Patienten deckt sich ungefähr mit den Ergebnissen der Patienten die in klinischen Studien zur Thrombolyse behandelt wurden, so dass die Thrombolyse nicht in einem zu hohen Maß eingesetzt wurde“, verdeutlicht Erstautor Dr. Gumbinger, Koordinator der AG Versorgungsforschung. “ Das Fehlen der Stroke Unit bedingt, dass nur 13 Prozent der rechtzeitig eintreffenden Patienten eine Thrombolyse erhalten. Das Land Baden-Württemberg kann ein gut ausgebautes Netz an Stroke Units vorweisen. Seit 2006 hat sich im Rhein-Neckar-Kreis eine teleneurologische Kooperation des Universitätsklinikums Heidelberg mit Kliniken in Sinsheim und Heppenheim etabliert.
