Brasiliens Kampf gegen Lepra
Die steigende Zahl der Lepra-Infektionen in Brasilien lässt die Nachfrage zum vermehrten Einsatz vom berüchtigten Schlaf- und Beruhigungsmittel Thalidomid unaufhaltsam steigen. Eine im Jahr 2015 im Fachjournal »Reproductive Toxicology« veröffentlichte statistische Erhebung durch Wissenschaftler brachte hervor, dass zwischen 2005 und 2010 knapp 5,9 Millionen Thalidomid-Tabletten an Leprapatienten ausgegeben wurden.
Diese Zahl steht außerdem in verdächtigem Zusammenhang mit 192 charakteristischen Geburtsfehlern im selben Zeitraum. Weitere Auswertungen von Geburtenrate und Lepraprävalenz untermauerten den Zusammenhang zwischen dem Absatz von Thalidomid-Tabletten und den körperlichen Missbildungen. Angesichts der statistischen Angaben der Deutschen Lepra- und Tuberkulosehilfe (DAHW), nach der sich in Brasilien jährlich 30.000 Menschen infizieren, ist der Einsatz von Thalidomid trotz der Gefahr für Nichtgeborene verständlich. Wiewohl seit 2003 strenge Auflagen zur Ausgabe des Medikaments bestehen, steht die Vermutung über eine unzureichende Kontrolle im Raum. Hinzu kommt der Verdacht, dass der verbreitete Analphabetismus in Brasilien die falsche Deutung des Warnsymbols einer durchgestrichenen Schwangeren begünstigt. Medienberichten zufolge wurde dieses Symbol in vielen Fällen als Verhütungsmittel gedeutet.
Die vermutlich durch Tröpfcheninfektion übertragbare Krankheit die mit Hautläsionen und dem Befall peripherer Nerven einhergeht, bedroht vor allem Menschen mit ausgeprägter Immunschwäche. Einst führte der hoffnungtragende Arzneistoff in den 50er Jahren zu zahlreichen Fällen von Embryopathien. Unter dem Begriff Contergan-Skandal verbreiteten sich Schreckensnachrichten über Geburten von etwa weltweit 10.000 missgebildeten Kindern. Das Mittel hatte sich zu seiner Zeit als Schlaf- und Beruhigungsmittel unter dem Namen Contergan folgenschwer etabliert. Dennoch ist das Mittel seit der Jahrtausendwende wieder wachsend im Einsatz. In Deutschland ist der Gebrauch von Thalidomid seit 2009 unter hohen Sicherheitsauflagen bei multiplem Myelom zugelassen. Aufgrund seiner tumorwachstums-, entzündungs- und blutneubildungshemmenden Wirksamkeit findet der Arzneistoff weltweit breiten Einsatz. So wird es in anderen Ländern auch bei Aids oder bestimmten Atoimmunerkrankungen eingesetzt.
Die Furcht vor einer Wiederkehr der verursachten Missbildungen ist scheinbar nicht so groß wie die vor der Lepra-Komplikation, der Immunreaktion Typ 2, genannt Erythema Nodosum Leprosum (ENL). Diese tritt vor allem bei der lepromatösen, einer unter vielen anderen Formen von Lepra auf. Sie führt zur Bildung von Immunkomplexen gegen das Microbacterium laprea und gehört nach Einschätzungen von Ridley und Jopling zum schwersten Verlauf. Gerade hier wird die antientzündliche und immunmodulierende Eigenschaft des Wirkstoffs hoch geschätzt und findet ihren effektiven Einsatz bei ENL. Das in die Nervenbahnen eingedrungene Bakterium beeinträchtigt den Tastsinn, wodurch die unbemerkten Verletzungen entstehen.Zu Entzündungen und Fieber kommt auch die Erblindung, welche durch Schädigungen am Sehnerv herbeigeführt wird. Der Kampf gegen Lepra zeichnet sich durch zwei hauptsächliche Ansätze aus. Während Ripley und Jopling sich der Struktur und Form der Infektionskrankheit widmen, steuert die WHO mit therapeutischen Maßnahmen dagegen und unterscheidet zwischen der paucibazillären, mit geringere Erregerlast und der multibazillären Lepra mit einer entsprechend höheren Erregerlast.
